Date of birth and account password if you are listed with Telstra as an Authorised Representative OR
Provide the "amount payable" and the "pay by" date of the last invoice you received for your account
Your ABN/ACN
Your Telstra account number handy (which can be found on the top right hand corner of your Telstra bill)
